Who We Are
Faith in Action-Caring Hearts, Helping Hands was founded in 2003 by a coalition of Rainbow Hospice,
Inc., Fort Healthcare and Watertown Memorial Hospital. Our Faith in Action program enables churches
and community groups to provide volunteer services to people in their area. Our goal is to help these
communities identify those in need, and provide them with the required help.

WHAT WE DO
Faith in Action provides neighborly assistance to elderly and disabled individuals to allow them to remain
independent in their homes. We provide this assistance through recruiting and training volunteers from
area congregations and the community. Some of our volunteer provided services include
companionship, meal preparation and delivery, respite care, yard work, rides to medical appointments,
grocery shopping, and minor home repairs and maintenance. Our services are always provided free of
charge to individuals living with difficulties due to aging or chronic illnesses. The individuals who receive
these services often experience improved quality of life, and are able to live in their homes longer than
would otherwise be possible.

Where we Serve
We currently operate volunteer programs in Watertown, Jefferson, Lake Mills, Palmyra, Johnson Creek,
and Rome. Our volunteers from these programs serve all of Jefferson County and surrounding
communities in Dane, Dodge, and Walworth counties. We currently have over 100 volunteers serving
more than 200 individuals.

How All of This is Possible
Faith in Action is able to provide all of these services on a limited budget. Two paid employees work a
combined total of thirty hours per week. Trained and dedicated volunteers provide all other services
and support. This program began with a seed grant from the Robert Wood Johnson Foundation. It
receives additional financial support from Rainbow Hospice Care, Helen Bader Foundation, Fort
Atkinson Hospital, Watertown Memorial Hospital, Tomorrow's Hope, United Way and private donations.
